When I have received your camp request I will pencil your campers into the appropriate weeks (provided there is space in the camp) and then send you the camp forms or you can download PDFs of the forms here.

• Emergency Information Form
• Agreement to Hold Harmless/Image Release Form
• Dispensing Medication Form

2. Once you have printed out and returned the camp forms to me along with payment, this confirms your spot.
